
PCPH Patient Journey

Patients seen at PCPH will first fill in essential details on a registration form.

Nursing staff will then record their basic physical parameters (including height, weight, temperature, blood pressure, and pulse rate).

When seen by the doctor, the patient's medical history and physical examination findings are recorded, with a detailed enquiry for any pertinent risk factors.

A tentative diagnosis is then arrived at, after which the treatment plan will be discussed with the patient, as well as any other family members (if present).

A comprehensive approach, including Investigations for diagnosis and various treatment methods, will be adopted, emphasizing non-invasive and cost-effective means.

The final plan will take into account the patients and their families' wishes. If these pose any potential danger(s), the downside will be fully explained to the patient for their reconsideration.

Investigations will then commence, beginning with equipment already available at the premises of PCPH, such as a resting electrocardiogram (ECG).

For testing that involves equipment not available at the office premises, such as Cardiac CT/ Cardiac MRI/ Cardiac Catheterization, appointments for these investigations will be made by our nursing staff according to availability.

Treatment will be initiated as appropriate, including medications that can be supplied on-site.

More intensive treatment may require admission to a hospital where close monitoring can be administered to ensure the patient can tolerate the effects of the treatment.

Hospital treatment will typically be required for the insertion of catheters, such as for cardiac catheterization, balloon surgery, etc.

After completion of testing and institution of initial treatment procedures, the patient, plus members of his family if desired, will be re-interviewed to discuss the progress of treatment and to plan further management as required.
